My dog doesn't seem well and idk what to do

So I have a 13 (ish) year old dog and for the past 4y she hasn't been herself fully and it's been getting worse since the start of this month she has lumps on her stomach and is almost always coffing and a few minutes ago I saw her Panting and looked like she couldn't breathe and idk what to do I'm 13 I have had her my hole life and my graperants hav been refusing to bring her to the vet since I had first pointed it out in 2020 and I'm concerned for her and idk how to help her my Nan said she would try to get her into the vet today but she has lied about it before and she seams to not care about her and I'm just scared something might happen to her
